About Your Medical Records at Winchester Hospital
Health Information Management at Winchester Hospital is responsible for maintaining patient medical records and the cancer registry.
Please contact Health Information Management at 781-756-2170 with requests for patient medical records, or see the information below. Please understand that because of our obligation to protect patient confidentiality, we cannot e-mail patient information.
How to Get Medical Records
To get copies of your medical records, you must send us a written request through the mail, authorizing Winchester Hospital to release your records. Please include the following information in your letter:
- Your name and address
- Your date of birth
- The time period (dates of service) for which you need your records
- The name of the person to whom we should release the medical records
- The reason why the medical records are needed
