Winchester Family Physicians Earn National Recognition for Health Care Model

Winchester Family Physicians, which is a member of the Winchester Physician Associates, has achieved Level-2 recognition for their Patient-Centered Medical Home (PCMH) model by the National Committee for Quality Assurance (NCQA).

PCMH is a model for care provided by medical practices that seeks to strengthen the doctor patient relationship by replacing episodic care based on illness with coordinated care and team relationships with the patient.The physician-led care team is responsible for providing all the patient’s health care needs and, when needed, arranges for appropriate care with other qualified clinicians.

“The Patient-Centered Medical Home model is another way we are improving primary care for our patients,” said Kevin Smith, President and CEO of Winchester Hospital. “Winchester Family Physicians is being recognized for changing their model of care to improve patient access and to make sure patients get the necessary care to stay healthy.”

This Level-2 recognition (the second highest level) ensures patients and their family members that the practice is patient-centric,focusing on a patient’s entire care and considering them part of the healthcare team.

Medical homes also emphasize enhanced care through open scheduling, expanded hours and communication between patients, clinicians and staff.
